In the movie Memento, how does Leonard's wife die?

If Sammy Jankis's story was actually Leonard's story, that means Leonard had this problem called 'anterograde amnesia' much before the attack by the two men. So, how does his wife actually die? The insulin overdose or murder by the two men? If its the insulin overdose, she would have been dead much before the attack, right?
